Information for the coordinator/host:

• The event's coordinator and/or host will receive a 25% discount on their Botox treatment for each event hosted.
• At least 10 confirmed guests are required.
• Same-day registration will be allowed if the time block permits individuals to join at the last minute.
• An additional AFSC employee will assist the injector to facilitate treatment.
• AFSC will use Eventbrite.com as a platform for RSVP, registration, and collecting a $50 deposit. A $50 deposit will be applied to the total treatment cost at the event's time.
• The event will be categorized as a PRIVATE event, meaning that ONLY IF YOU HAVE THE LINK, YOU ARE ABLE TO REGISTER. AFSC will post the event, including all necessary information on AFSC’s behalf and the hosting parties, including graphics that are shareable on social media platforms. AFSC can also provide a printable PDF for the specific event for displaying inside the establishment. The HOST/COORDINATOR will promote this event as they desire, NOT AFSC. The attendees list will be BLIND to preserve privacy.
• Attendees will receive a full refund if their registration is canceled 7 days before the event. Otherwise, their deposit will be used as a credit toward any purchase at About Face Skin Care for future treatments.
• If AFSC must cancel an event due to unforeseen circumstances, all attendees will receive a full refund or (at the attendee’s discretion) be rescheduled for another event/time.
• Any amount due to AFSC at the event will be collected. We do not bill.
• All attendees will be confirmed by AFSC 48 hours (about 2 days) before the event via text and/or email. In addition, and for efficiency purposes, AFSC will attempt to collect any intake forms that are required by law or the Georgia Board of Composite Medicine before the event.
• All attendees will be required to sign an “INFORMED CONSENT” at time of treatment during the event. HIPPA Compliant photos will also be required as dictated by our risk management.
• Our malpractice provider fully insures AFSC as a mobile treatment provider.
• The event BLOCKED TIME is reserved for treating all attendees. AFSC cannot guarantee a specific time within the blocked time. However, AFSC will do its best to accommodate attendees with time constraints. Generally, AFSC allows 15 minutes per treatment per attendee.
• Alcohol is not recommended at this event due to bruising effects. If an attendee appears to be under the influence, AFSC reserves the right to refuse treatment.
• Any attendee who has post-treatment issues will have to be seen inside our practice at any of our locations. It is HIGHLY advisable that all attendees who have treatment follow up with our practice after 15 days (about 2 weeks) post treatment.
• AFSC is not responsible for delays due to weather, traffic, construction, directions, or any other instances beyond our control.
• AFSC cannot guarantee certain practitioners, although we will do our best. However, AFSC reserves the right to substitute requested practitioners should a conflict arise.
• AFSC reserves the right to refuse treatment for ANY reason.
